A five-minute assessment to hand a family at discharge
For discharge planners, social workers, therapists, care managers, and physicians. Free, and the answer comes before any form.
What it is
The adult child answers short questions about a parent who lives alone: everyday tasks, the home, falls, memory, medicines, and who is nearby. The family gets one of three plain results and an explanation by area. The rules are published in full and clinically reviewed.
What it will not do
It is not a diagnosis and not medical advice. It does not replace your assessment, your discharge plan, or your duty to act on an immediate concern. The first screen sends anyone in danger to 911.
What comes back to you
Nothing about the family. You share a generic link, send us no patient information, and receive no answers, result, or contact details.
A referral link may carry a source code that counts visits. It holds no health information and cannot show you who answered.
Questions professionals ask
Does this make us business associates?
No. A generic referral in which you send no patient information and receive no assessment data does not create a business-associate relationship. Parents Home Care does not create or maintain the result on behalf of the referring provider. If a family shares a read-only result link with you, that is their choice. Review the HHS health-app guidance with counsel for each institutional use.
Will the family be sold to a placement service?
No. Any placement introduction is optional and carries only a random reference code, never a name, result, or answers. How we make money.
